申請人(父或母)必須為澳門居民及天主教教友
孕期未滿 22 周的自然流產胎兒
Applicant (father or mother) must be a Macau resident and a Catholic
Fetus miscarried at less than 22 weeks
所需文件 DOCUMENTS REQUIRED
申請人(父及母)之澳門居民身份證
父母一方之領洗紙
結婚證明書
流產證明文件(如有)
Applicants’ (father and mother) Macau identity cards
Baptismal certificate of one parent
Marriage certificate
Proof of miscarriage (if any)
申請流程 APPLICATION PROCEDURE
1) 請下載及填妥申請表(按此下載申請表)
並電郵至cdvadmin@catholic.org.mo;
2) 由澳門教區生命委員會專員約見以核實資料(帶備上述所需文件之正本)；
3) 經審批後獲發同意書；
4) 申請人帶備同意書到天主教殯儀館，完成立牌匾手續及繳付相關費用。

2. The eligible applicant will be arranged for an interview to verify the information.
(present with the above- mentioned required documents)
3. An authorized letter of approval will be issued for applicant complied with the conditions.
4. Proceed to the Diocesan Funeral Home for the name plaque engraved and complete payment.
天使紀念碑牌匾
收費：澳門幣3,500元 (包括管理費澳門幣3,000元及立碑費澳門幣500元)
尺寸：149mm X 99mm
註：1) 收費為一次性費用，使用期為30年。
2) 牌匾不可更換及轉讓。
3) 牌匾內容必須依照澳門天主教殯儀館逾越園所訂立的格式。
NAME PLAQUE OF THE WALL OF NAMES
Fee: Mop3500.00 ( Includes Administration fee Mop 3,000 and Name plaque fee Mop 500)
Size: 149mm X 99mm
Notes: 1) One time fee for the period of 30 years.
2) Name plaque is non- exchangeable nor transferable.
3) Content of the inscription must follow the format provided.
